This checklist identifies the information you'll need to complete the GIF. An asterisk (*) means the information is required. Once you've gathered this information, just log in, go to My GuideStar, and click Update my nonprofit report. You'll be asked to provide your Employer Identification Number (EIN).

 * Please list up to 3 major accomplishments from your past fiscal year and the date it ended. Please limit your response to 20 words (200 characters) per accomplishment.

Objectives

 * Please list up to 3 major operating objectives from your current fiscal year and the date it began. Please limit your response to 20 words (200 characters) per objective.

Keywords

 Choose keywords that describe your mission or programs (100 characters maximum). Put a comma between keywords.
